Ultrasound elastography is not accurate for diagnosing juvenile idiopathic inflammatory myopathy in children. Magnetic resonance imaging (MRI) remains the standard for detecting myositis, as elastography showed low sensitivity and specificity.

Background:

  • Juvenile idiopathic inflammatory myopathy (JIIM) is a rare, debilitating childhood condition.
  • MRI is the current standard for diagnosing JIIM and monitoring treatment response.
  • The diagnostic performance of ultrasound (US) elastography for JIIM in children has not been previously studied.

Purpose of the Study:

  • To evaluate the effectiveness of compression-strain US elastography in identifying active myositis in children diagnosed with JIIM.
  • To compare the diagnostic accuracy of US elastography with MRI in pediatric patients with JIIM.

Main Methods:

  • Children with confirmed JIIM underwent both non-contrast MRI and US elastography of the quadriceps.
  • Active myositis on MRI was defined by increased signal on T2-weighted images.
  • US elastography findings were compared to MRI results and clinical assessments of active disease.

Main Results:

  • MRI identified active myositis in 10 of 18 pediatric patients.
  • US elastography showed abnormal findings in 4 cases with abnormal MRI and 4 with normal MRI.
  • Elastography demonstrated a significant association with muscle echogenicity (P < 0.001) but not with MRI or clinical disease activity.

Conclusions:

  • Compression-strain US elastography is not a reliable method for detecting active myositis in children with JIIM.
  • US elastography cannot substitute MRI as the primary imaging modality for diagnosing myositis in this population.
  • Further research is needed to clarify the clinical significance of US elastography findings in pediatric myositis.
