Background:

Children's public health coverage is crucial for access to care.
Retention in Medicaid and Children's Health Insurance Program (CHIP) is a key policy concern.
Program churning and coverage gaps impact vulnerable populations.

Purpose of the Study:

To analyze state progress in retaining children in public coverage.
To examine public coverage churning and transfer-related gaps in 10 states.
To identify variations in retention and churning across states.

Main Methods:

Utilized administrative data from 10 states over a five-and-a-half-year period.
Analyzed children's transitions between Medicaid and CHIP.
Assessed rates of disenrollment, re-enrollment, and coverage gaps.

Main Results:

Significant movement between Medicaid and CHIP occurred, with up to 40% experiencing coverage gaps.
Approximately 21% of Medicaid and 10% of CHIP disenrollees returned within 7 months (churning).
Substantial state-level differences in churning and transfer gaps were observed.

Conclusions:

State and program variations in public coverage churning persisted.
Simplifying renewal processes, especially for Medicaid, is essential.
Improved inter-program coordination and transfer policies are needed to reduce coverage gaps.
