Summary

Malignancy within fibroadenomas (FAs) diagnosed by core biopsy is rare. Growing FAs also show a very low risk of cancer, suggesting routine follow-up may not be necessary for benign diagnoses.

Background:

  • Fibroadenomas (FAs) are common breast tumors in young women.
  • Core biopsy is increasingly used for FA diagnosis, necessitating imaging follow-up.
  • Management guidelines for growing FAs require re-evaluation.

Purpose of the Study:

  • To determine the incidence of epithelial abnormalities in FAs diagnosed by core biopsy.
  • To assess the risk of malignancy in enlarging FAs.
  • To re-evaluate the management of growing FAs.

Main Methods:

  • Retrospective review of 2062 nodular FAs diagnosed over 19 years.
  • Identification of 83 FAs that enlarged after initial core biopsy.
  • Analysis of subsequent management and pathology for enlarging FAs.

Main Results:

  • Malignancy (atypia, in situ, or invasive) was found in 0.58% of initial core biopsies.
  • None of the 83 enlarging FAs showed malignancy; two were benign phyllodes tumors.
  • The incidence of cancer in growing FAs was 0% in this series.

Conclusions:

  • Malignancy adjacent to or within core-biopsied FAs is rare.
  • The risk of cancer in growing FAs is negligible, questioning the need for specific follow-up.
  • Excision is recommended for FAs with abnormal epithelial findings; otherwise, routine follow-up is likely unnecessary.
