A rare soft tissue aneurysmal bone cyst (ABC) occurred in a 41-year-old female, presenting as a shoulder mass. This case highlights the importance of considering soft tissue ABC in adults, with successful surgical excision and no recurrence.
Area of Science:
- Orthopedics
- Radiology
- Pathology
Background:
- Soft tissue aneurysmal bone cyst (ABC) is exceptionally rare, particularly in adults over 40.
- Literature reports approximately 20 cases, with only 3 in patients older than 40.
Observation:
- A 41-year-old female presented with atraumatic chest pain, later developing a palpable left shoulder mass.
- Imaging revealed a large mass with rim calcification, hypodense areas, and thin septations.
- MRI demonstrated hemorrhagic cystic spaces with fluid-fluid levels and enhancing septations.
Findings:
- Initial biopsy interpretation was cystic myositis ossificans, revised to soft tissue ABC on final review.
- Surgical excision was performed.
- A 3-year post-operative radiograph showed no evidence of recurrence.
Implications:
- This case expands the understanding of soft tissue ABC presentation in adults.
- Accurate diagnosis requires careful pathological review, distinguishing it from other cystic lesions.
- Successful surgical management is demonstrated, with long-term follow-up confirming no recurrence.

Bone Disorders
Bone deposition is also affected by the levels of sex hormones like estrogen and testosterone that promote osteoblast activity and bone matrix synthesis. When the level of these hormones decreases due to aging, it causes a reduction in bone deposition. As a result, bone resorption by osteoclasts...

Aneurysm I: Introduction
Compact Bone
Compact bone, also called cortical bone, is the denser, stronger of the two types of bone tissue. It is found under the periosteum and in the diaphyses of long bones, where it provides support and protection. The microscopic structural unit of compact bone is called an osteon, or haversian system. Each osteon is composed of concentric rings of calcified...
Gross Anatomy of Bone
The diaphysis is the tubular shaft that runs between the proximal and distal ends of the bone. The walls of the diaphysis are composed of dense and hard compact bone made of numerous osteons — the functional unit of the compact bone. The hollow region in the diaphysis is called the medullary cavity, which harbors the bone marrow. In infants and children, this marrow cavity is filled with red marrow, whereas in...
