A rare case of sarcoidosis with ocular involvement in a Chinese woman is presented. Diagnosis was confirmed via lung biopsy, and ocular symptoms resolved with steroid treatment.

    Area of Science:

    • Ophthalmology
    • Pulmonology
    • Pathology

    Background:

    • Sarcoidosis is a multisystem inflammatory disease characterized by noncaseating granulomas.
    • Ocular involvement in sarcoidosis can lead to significant vision impairment.
    • This report details a unique presentation of sarcoidosis with extensive ocular manifestations.

    Observation:

    • A 28-year-old Chinese woman presented with cough and bilateral hilar lymphadenopathy, diagnosed as sarcoidosis via transbronchial lung biopsy.
    • Eight months later, she developed blurred vision due to bilateral ocular inflammation, including conjunctival masses, keratic precipitates, iridocyclitis, vitreous opacity, and retinal periphlebitis.

    Findings:

    • Biopsy of conjunctival masses confirmed noncaseating granulomas, typical of sarcoidosis.

    Related Experiment Videos

  • This represents the only reported case of ocular sarcoidosis among 10 sarcoidosis cases at the institution over 20 years.
  • Implications:

    • Highlights the importance of considering sarcoidosis in patients with unexplained ocular inflammation.
    • Topical steroid therapy can effectively manage ocular sarcoidosis, leading to visual improvement.
