Summary

Many relapsing-remitting multiple sclerosis patients report frequent walking ability changes. These individual fluctuations, measured by the MSWS-12, were significant over two years, despite stable group scores.

Background:

  • Patient-reported outcomes (PROs) are vital for assessing multiple sclerosis (MS) disability and treatment effectiveness.
  • The 12-item Multiple Sclerosis Walking Scale (MSWS-12) is a key PRO for evaluating disease impact on walking.

Purpose of the Study:

  • To investigate longitudinal changes in walking ability using the MSWS-12.
  • To analyze walking ability fluctuations in patients with relapsing-remitting MS and moderate-to-severe disability.

Main Methods:

  • A cohort of 108 relapsing-remitting MS patients completed the MSWS-12 every 6 months for 2 years.
  • Concurrent self-reported measures included the Multiple Sclerosis Impact Scale and Patient Determined Disease Steps scale.

Main Results:

  • Approximately 80% of patients reported changes in walking ability at the individual level across all time points.
  • Individual MSWS-12 score changes exceeded proposed minimal clinically important differences.
  • Individual walking changes correlated more with physical than psychological impact scores, and weakly with Patient Determined Disease Steps.

Conclusions:

  • Frequent fluctuations in self-reported walking ability are a novel finding in MS patients.
  • These individual-level changes warrant further investigation to understand their clinical significance.
