Antifungal Agents01:15

Antifungal Agents

89
Amphotericin B is a broad-spectrum antifungal agent that exploits structural differences between fungal and mammalian cell membranes. Its amphipathic structure—featuring a hydrophobic polyene-lactone ring and a hydrophilic region containing mycosamine and carboxylic acid groups—enables selective binding to ergosterol, a sterol predominantly found in fungal plasma membranes. This selective interaction underlies the drug’s antifungal activity, although weak binding to...

Skin Diseases and Disorders

6.3K
Skin is the first line of defense and encounters a variety of microbes. Some pathogenic strains are often the cause of a broad range of infections of the skin and other body systems. These conditions can affect people of all ages and may have different causes, including genetic factors, infections, autoimmune reactions, environmental factors, and lifestyle choices.
Gram-positive Staphylococcus spp. and Streptococcus spp. are responsible for many of the most common skin infections. However, many...

The Skin Microbiota01:27

The Skin Microbiota

80
The human skin serves as a complex ecosystem inhabited by a diverse community of microorganisms, including bacteria, fungi, and viruses. This microbiome plays a critical role in maintaining skin health and defending against pathogenic invaders. The composition of microbial communities varies significantly across different regions of the body, influenced primarily by the local levels of moisture and sebum.Regional Variation in Skin MicrobiotaCutibacterium acnes predominantly colonizes sebaceous...

Accessory Structures of the Skin: Sebaceous Glands

5.7K
A sebaceous gland is a type of oil gland found almost all over the skin ( except palms and soles) and helps lubricate and waterproof the skin and hair. Most sebaceous glands are associated with hair follicles. They generate and excrete sebum, a mixture of lipids, onto the skin surface, thereby naturally lubricating the dry and dead layer of keratinized cells of the stratum corneum, keeping it pliable.
These glands that produce the oils on the skin and hair are holocrine glands. The mature...

Hand hygiene01:23

Hand hygiene

6.7K
Asepsis is the practice of preventing or breaking the chain of infection. The nurse employs aseptic techniques to prevent the spread of microorganisms and reduce the risk of diseases. Hand hygiene is the cornerstone of aseptic techniques and is classified into medical and surgical asepsis. Medical asepsis includes hand hygiene and the use of gloves. Surgical asepsis, or the sterile technique, refers to practices that render and keep objects and areas free of microorganisms.

Anthelminthic Agents01:15

Anthelminthic Agents

83
Anthelmintic drugs differ significantly from antiparasitic therapies targeting protozoa, primarily due to differences in parasite biology. Whereas most protozoal treatments act on proliferating cells, anthelmintics are typically directed against mature, nonproliferative helminths. The therapeutic approach considers the helminth's reliance on neuromuscular coordination, glucose metabolism, and microtubular integrity for survival, reproduction, and localization within the host. Most anthelmintics...

Summary

Topical antifungals like ketoconazole and ciclopirox effectively treat seborrhoeic dermatitis compared to placebo. Ketoconazole showed fewer side effects than steroids, but quality of life outcomes remain unassessed.

Area of Science:

  • Dermatology
  • Pharmacology
  • Evidence-based medicine

Background:

  • Seborrhoeic dermatitis is a prevalent chronic inflammatory skin condition affecting the scalp and face.
  • Treatment commonly involves antifungals, steroids, and other agents.

Purpose of the Study:

  • To evaluate the efficacy of antifungal agents for facial and scalp seborrhoeic dermatitis in adolescents and adults.
  • To assess antifungal effectiveness in patients with HIV/AIDS.

Main Methods:

  • Systematic review and meta-analysis of 51 randomized controlled trials (9052 participants).
  • Searched multiple databases up to December 2014.
  • Assessed primary outcomes of symptom clearance and quality of life.

Main Results:

  • Ketoconazole and ciclopirox demonstrated superior efficacy over placebo in treating seborrhoeic dermatitis.
  • Ketoconazole showed comparable remission rates to steroids but with significantly fewer side effects.
  • Evidence for other antifungals, long-term effects, and quality of life impact was limited.

Conclusions:

  • Ketoconazole and ciclopirox are effective topical treatments for seborrhoeic dermatitis.
  • Further high-quality research is needed to assess long-term outcomes and quality of life.
  • Current evidence supports ketoconazole's favorable side effect profile compared to steroids.
