Summary

Hip stability in posterior wall acetabular fractures is challenging to determine. Fracture exit point location is a key radiographic marker for stability, while wall size is not a reliable indicator.

Background:

Purpose of the Study:

  • To identify radiographic predictors of hip stability in posterior wall acetabular fractures.
  • To evaluate the role of femoral head coverage, acetabular version, fracture location, and history of dislocation in determining hip stability.

Main Methods:

  • Retrospective review of 185 patients with isolated unilateral posterior wall acetabular fractures.
  • Dynamic stress fluoroscopic examination under anesthesia to assess hip stability.
  • Radiographic measurements including wall size and fracture exit point relative to the acetabular dome.

Main Results:

  • Hip stability was assessed via examination under anesthesia (EUA), classifying 116 hips as stable and 22 as unstable.
  • Statistically significant differences were found in Moed and Keith wall size measurements and the cranial exit point of the fracture.
  • Unstable hips had smaller wall sizes (<20%) and a lower average cranial exit point from the dome (5.0 mm vs. 9.5 mm).

Conclusions:

  • Fracture location, specifically the exit point relative to the acetabular dome, is a valuable radiographic marker for assessing hip stability.
  • Wall size less than 20% is not a reliable indicator of hip instability in these fractures.
