Drug Dosing in Renal Diseases: Estimation of Glomerular Filtration Rate Based on Serum Creatinine Concentration

315
Glomerular filtration rate (GFR) can be estimated from serum creatinine using the modification of diet in renal disease (MDRD) formula or the chronic kidney disease–epidemiology collaboration (CKD–EPI) equation. Both methods are widely used in clinical practice to assess kidney function and guide treatment decisions. The MDRD equation does not require weight or height measurements and is normalized to the body surface area of 1.73 m², considered the average adult surface area.

Summary

Resting metabolic rate (RMR) estimation models using air displacement plethysmography (ADP) may underpredict caloric needs. The Doré et al. (1982) model showed the best agreement with indirect calorimetry (IC) measurements.

Area of Science:

  • Metabolic research
  • Body composition analysis
  • Physiological measurements

Background:

  • Resting metabolic rate (RMR) estimation is crucial for nutritional assessment and is often predicted using regression equations.
  • Air displacement plethysmography (ADP) is a common method for body composition assessment, with associated RMR predictive models.
  • The accuracy of these ADP-based RMR models compared to indirect calorimetry (IC) requires thorough investigation.

Purpose of the Study:

  • To evaluate the agreement between RMR estimation models utilizing air displacement plethysmography (ADP) measures and a criterion standard gas exchange indirect calorimetry (IC) system.
  • To identify which validated ADP-based RMR predictive models demonstrate the best agreement with IC measurements.

Main Methods:

  • Sixty-six participants (25 men, 41 women) underwent RMR measurements using both IC and ADP within a 10-minute interval.
  • Nine validated RMR predictive models using ADP measures were statistically compared against IC RMR estimates.
  • Analysis of variance (ANOVA) with multiple comparisons and Bland-Altman analysis were employed to assess agreement.

Main Results:

  • The Nelson (1992) model significantly underpredicted RMR when compared to IC (p < 0.001).
  • The Doré et al. (1982) model demonstrated the highest agreement with IC RMR measurements (p = 0.907), indicating superior predictive accuracy.

Conclusions:

  • Current RMR estimation models based on ADP measures may underestimate total daily caloric requirements.
  • The Doré et al. (1982) model is recommended for more accurate RMR prediction in diverse populations when using ADP-derived data.
