[A case report: primary nasal tuberculosis].

    Summary

    Nasal tuberculosis is a rare condition that can mimic other diseases like sarcoidosis. Prompt diagnosis through repeated biopsies and cultures is crucial for effective treatment and recovery.

    Area of Science:

    Background:

    • Nasal and paranasal sinus masses can present with varied etiologies, often mimicking inflammatory or neoplastic conditions.
    • Epithelioid granulomas without caseous necrosis on initial biopsy can suggest sarcoidosis, complicating the diagnostic pathway.

    Observation:

    • A 62-year-old female presented with lacrimation and nasal swelling, revealing granulation tissue and a mass in the nasal cavity and ethmoid sinuses.
    • Imaging showed facial invasion and nasal bone erosion, while initial histology indicated epithelioid granuloma suggestive of sarcoidosis.

    Findings:

    • Repeated biopsies over 11 months showed persistent epithelioid granulomas, initially negative for Mycobacterium tuberculosis via PCR.
    • However, Mycobacterium tuberculosis was eventually detected by culture, leading to a diagnosis of primary nasal and paranasal sinus tuberculosis.
