Background:

Nexplanon is a long-acting reversible contraceptive implant releasing etonogestrel for 3-year contraception.
The implant contains barium sulfate for X-ray visibility, a component rarely associated with adverse reactions.

Observation:

A 39-year-old woman presented with a local reaction attributed to the barium sulfate within her Nexplanon implant.
Medical treatment did not alleviate symptoms; only surgical removal of the contraceptive device provided relief.

Findings:

The patient experienced complete symptom resolution within 72 hours post-implant removal.
This case represents one of only three documented instances of barium sulfate allergy linked to Nexplanon.

Implications:

Healthcare providers should consider barium sulfate allergy in patients experiencing unexplained local reactions to Nexplanon.
Further pharmacovigilance is warranted to understand the incidence and management of rare adverse events associated with contraceptive implants.
