Adverse reactions to contrast agents during micturating cystourethrography (MCU) are rare but can be severe. This case highlights the need for caution and preparedness during MCU procedures, especially in at-risk patients.

Background:

  • Parenteral contrast agent reactions are well-documented.
  • Reactions during micturating cystourethrography (MCU) are less common.
  • Absorption of contrast media through bladder mucosa is possible.

Observation:

  • A significant adverse reaction occurred during ionic contrast material MCU.
  • Neither urinary reflux nor traumatic catheterization was implicated.
  • The patient had a history of adverse reaction to IV contrast media.

Findings:

  • Significant adverse reactions can occur during MCU procedures.
  • Appropriate resuscitation facilities are essential during MCU.
  • Contrast media absorption via bladder mucosa can lead to systemic reactions.

Implications:

  • Review the necessity of MCU in patients with prior contrast reactions.
  • Consider nuclear medicine MCU as an alternative.
  • If radiologic MCU is required, consider hospital setting, nonionic contrast, and steroid premedication.
