Summary

Benign thyroid enlargement can cause phrenic nerve palsy, a previously unreported complication. Prompt surgery for large goiters may prevent or reduce the severity of this condition, avoiding permanent nerve damage.

Background:

  • Thyroid goiters, especially substernal ones, can compress adjacent structures, causing dysphagia and dyspnea.
  • Phrenic nerve injury from goiter compression is a rare but significant complication.

Observation:

  • The phrenic nerve may be stretched by goiters in the neck.
  • Compression near the first rib is a more likely site for nerve injury.
  • Unilateral palsy often goes unnoticed due to lack of symptoms.

Findings:

  • Bilateral phrenic nerve palsy can lead to severe dyspnea and pulmonary insufficiency.
  • Elderly patients with cardiopulmonary disease are particularly vulnerable.

Implications:

  • Early surgical intervention for goiters is crucial to prevent phrenic nerve palsy.
  • Timely treatment can avoid permanent nerve damage and improve patient outcomes.
