Metaplastic conus meningioma and its applied aspects

This case report highlights a rare calcified metaplastic meningioma in the lumbar spine. Early detection and careful surgical dissection are crucial for preventing complications in spinal tumor cases.

Background:
- Spinal meningiomas represent 25% of all spinal tumors, typically occurring in the dorsal spine of middle-aged females.
- Intradural-extramedullary meningiomas are usually surgically accessible, but lumbar presentations can pose challenges.
Observation:
- A 55-year-old patient presented with backache due to a calcified lesion at the L1 level, initially suspected to be an ependymoma.
- Intraoperative findings revealed partial calcification, and histopathology confirmed a metaplastic meningioma with calcification.
Findings:
- Calcification is common in meningiomas, but frank bony metaplasia is exceptionally rare.
- The patient experienced transient hip weakness, which fully resolved post-operatively.
Implications:
- Lumbar meningiomas, especially those with calcification and bony metaplasia, require meticulous surgical dissection to avoid neural structure damage.
- Increased awareness of these rare presentations can improve surgical outcomes and prevent future complications.
