Background:

  • Technical and nontechnical skills are essential in surgery but often taught separately.
  • Nontechnical skills include situational awareness, communication, decision making, teamwork, and leadership.
  • Integrating these skill sets into surgical training curricula is lacking.

Purpose of the Study:

  • To evaluate the relationship between technical and nontechnical skills in a simulation-based surgical environment.
  • To determine if specific nontechnical skill components are particularly relevant to technical performance in ureteroscopy.

Main Methods:

  • A prospective analysis of data from a comparative study of simulation vs. non-simulation training in ureteroscopy.
  • 32 novice participants were assessed on technical skills (time, OSATS, checklist) and nontechnical skills (NOTSS).
  • Pearson correlation coefficient was used to analyze the data for the whole and individual cohorts.

Main Results:

  • A strong negative correlation was found between time to completion and Nontechnical Skills for Surgeons (NOTSS) scores (r = -0.75).
  • Strong positive correlations were observed between NOTSS scores and Objective Structured Assessment of Technical Skills (OSATS) (r = 0.89) and task-specific checklist scores (r = 0.91).
  • All individual nontechnical skill components strongly correlated with technical skill parameters, irrespective of training.

Conclusions:

  • A significant correlation exists between technical and nontechnical surgical performance, independent of prior training.
  • All assessed nontechnical skills are important for technical performance.
  • Surgical training and assessment should integrate both technical and nontechnical skills within a unified curriculum.
