Expansive cervical laminoplasty using miniplates provides the greatest spinal canal expansion compared to other lamina spacers. This surgical technique offers excellent widening for improved outcomes.

Area of Science:

  • Neurosurgery
  • Spinal Surgery
  • Orthopedic Surgery

Background:

  • Cervical laminoplasty is a surgical procedure to decompress the spinal cord.
  • Various lamina spacers are used to maintain spinal canal expansion post-surgery.
  • Comparing the efficacy of different spacers is crucial for optimizing surgical outcomes.

Purpose of the Study:

  • To introduce a miniplate surgical method for expansive cervical laminoplasty.
  • To compare the spinal canal expansion rates achieved with miniplates versus other lamina spacers (Hydroxyapatite and Centerpiece).

Main Methods:

  • Retrospective analysis of 39 patients undergoing expansive cervical laminoplasty between 2008 and 2011.
  • Evaluation of lamina spacer type, pre- and postoperative spinal canal areas via CT scans.

  • Calculation of spinal canal expansion rates using Photoshop CS3®.
  • Main Results:

    • Miniplates achieved the highest expansion rate (76.5%) compared to Hydroxyapatite (49.8%) and Centerpiece (50.6%).
    • Miniplate fixation resulted in a 90° box-shaped widening of the laminae, easily verifiable by AP X-ray.
    • No complications were reported with the use of miniplates.

    Conclusions:

    • Box-shaped laminoplasty utilizing miniplates is the most effective method for achieving maximal spinal canal expansion among the implants studied.
    • Miniplates offer a superior option for spinal canal widening in cervical laminoplasty.
