Summary

Systemic lupus erythematosus (SLE) can cause lupus enteritis during pregnancy, presenting as acute abdomen. Magnetic resonance imaging aids diagnosis, and prompt treatment with steroids can lead to successful delivery.

Background:

  • Systemic lupus erythematosus (SLE) exacerbations during pregnancy are common, but lupus enteritis is rarely reported in pregnant patients.
  • Autoimmune diseases like SLE can affect multiple organ systems, including the gastrointestinal tract.

Observation:

  • A 29-year-old pregnant woman with a history of pure red cell aplasia and positive anti-Ro/SS-A and anti-La/SS-B antibodies presented with acute abdomen.
  • Clinical presentation included mild increases in anti-DNA antibodies and decreased complement levels.
  • Abdominal imaging revealed significant ascites and small intestinal wall edema.

Findings:

  • The patient was diagnosed with lupus enteritis based on clinical and imaging findings.
  • Treatment with prednisolone resulted in symptom improvement.
  • The patient delivered a healthy infant at 37 weeks gestation.

Implications:

  • Lupus enteritis should be considered in pregnant patients with autoimmune diseases presenting with acute abdominal symptoms.
  • Magnetic resonance imaging (MRI) is a valuable diagnostic tool for lupus enteritis in pregnancy.
  • Early diagnosis and management of lupus enteritis can lead to favorable maternal and fetal outcomes.
