Summary

Gallstone ileus, a rare bowel obstruction, can occur even years after gallbladder removal. This case reveals a unique cause of gallstone ileus despite a prior cholecystectomy.

Background:

Gallstone ileus is a rare complication of gallstones, typically involving a fistula between the gallbladder and the gastrointestinal tract.
It occurs when a gallstone obstructs the bowel, leading to ileus.
While often associated with an intact gallbladder, cases post-cholecystectomy are documented.

Observation:

A patient presented with symptoms suggestive of small bowel obstruction.
Computed tomography indicated a gallstone causing the obstruction.
The patient had undergone a cholecystectomy over 40 years prior to this presentation.

Findings:

Exploratory laparotomy confirmed a gallstone obstructing the small bowel, leading to gallstone ileus.
