Painless giant cell thyroiditis is an atypical subacute thyroiditis subtype presenting with fever, weight loss, and elevated erythrocyte sedimentation rate (ESR). This condition should be considered in patients with unexplained pyrexia and weight loss.

Area of Science:

  • Endocrinology
  • Pathology

Background:

  • Subacute thyroiditis typically presents with neck pain, gland enlargement, and thyrotoxicosis.
  • Granulomatous inflammation is a hallmark of certain thyroiditis types.

Observation:

  • A 50-year-old patient presented with prolonged fever, weight loss, malaise, and elevated erythrocyte sedimentation rate (ESR).
  • Thyroid cytology revealed granulomas with giant cells, characteristic of granulomatous thyroiditis.

Findings:

  • The patient lacked typical symptoms of subacute thyroiditis, specifically neck pain and thyrotoxicosis.
  • This presentation suggests a variant of subacute thyroiditis, termed painless giant cell thyroiditis.

Implications:

  • Painless giant cell thyroiditis should be considered in the differential diagnosis of undiagnosed pyrexia, weight loss, and elevated ESR.

  • Recognizing this subtype is crucial for appropriate patient management and avoiding misdiagnosis.
