Background:

  • Mediastinal paragangliomas are rare neuroendocrine tumors.
  • Accurate diagnosis relies on integrating imaging findings with histopathology.

Observation:

  • A middle mediastinal mass was incidentally detected on CT in a 48-year-old woman.
  • MRI revealed iso-signal intensity on T1, high signal intensity on T2, and intermediate signal on DWI (ADC=1.72 × 10(-3) mm(2)/s).
  • Dynamic contrast-enhanced CT showed rapid peak and washout enhancement.

Findings:

  • (18)F-FDG-PET demonstrated increased uptake (SUVmax=7.88).
  • (123)I-meta-iodobenzylguanidine scintigraphy also showed abnormal tumor uptake.
  • Histopathology confirmed hypervascularity, rich stroma, and neuroendocrine markers.

Implications:

  • This case underscores the utility of multimodal imaging in diagnosing mediastinal paragangliomas.
  • Understanding the radiologic features aids in differentiating these tumors from other mediastinal masses.
