Background:

Vertebral metastasis is common, but disc metastasis is rare.
Understanding the mechanisms of tumor spread to the spine is crucial.

Purpose of the Study:

To investigate the reasons for the rarity of malignancy metastasis into the intervertebral disc.
To identify pathways of tumor invasion from vertebrae to discs.

Main Methods:

Histopathologic examination of 14 vertebral columns with metastases.
Analysis of 616 intervertebral disc specimens.

Main Results:

Identified three invasion pathways: direct infiltration, subligamentous spread, and hematogenous seeding.
Cartilaginous endplate and high intradiscal pressure act as barriers.

Conclusions:

The cartilaginous endplate and intradiscal pressure inhibit tumor spread to the disc.
Age-related vascularization of the disc may rarely facilitate hematogenous metastasis.
