Summary

Deep brain stimulation (DBS) offers favorable results for chronic pain, particularly when other treatments fail. This neuromodulation technique shows promise for various pain syndromes, including failed back surgery syndrome and cluster headaches.

Area of Science:

  • Neurosurgery
  • Pain Management
  • Neurology

Background:

  • Deep brain stimulation (DBS) has been utilized for chronic pain management since the 1970s.
  • It is considered an option for patients refractory to conventional treatments like medications and surgery.

Purpose of the Study:

  • To review the literature on the efficacy of DBS in treating diverse chronic pain syndromes.
  • To identify successful treatment targets and pain conditions with the highest response rates.

Main Methods:

  • A literature review was conducted using the PubMed database.
  • Studies from the last 30 years were included, with a focus on the last 10 years, prioritizing conditions with high success rates and specific targets.

Main Results:

  • DBS demonstrates favorable outcomes for chronic pain, especially in cases resistant to other interventions.
  • Conditions like failed back surgery syndrome, phantom limb pain, and neuropathic pain show response, with nociceptive pain generally responding better than neuropathic pain.
  • Cephalalgias, particularly cluster headaches, exhibit promising results with DBS treatment.

Conclusions:

  • Deep brain stimulation is a viable therapeutic option for select chronic pain conditions.
  • Despite being investigational in the USA, DBS shows promising success rates in patients with treatment-resistant pain.
