Tissue expansion can increase free flap size for pediatric reconstructions. Expanded parascapular free flaps successfully reconstructed lower limb defects in children, doubling skin area with normal limb growth and no donor site issues.

Area of Science:

  • Plastic Surgery
  • Regenerative Medicine
  • Pediatric Surgery

Background:

  • Controlled tissue expansion is a reconstructive technique to increase local tissue availability.
  • Expanding free flaps before elevation enhances their surface area, particularly beneficial for pediatric patients with limited donor site tissue.

Observation:

  • Four pediatric cases with extensive lower limb cutaneous defects were treated.
  • Parascapular free flaps were expanded prior to elevation and used for reconstruction.

Findings:

  • Expanded flaps demonstrated an approximate doubling of skin surface area.
  • Reconstructions resulted in complete healing at recipient sites.
  • Affected limbs exhibited normal growth, and donor sites had no morbidity.

Related Experiment Videos

Implications:

  • Pre-operative flap expansion is a viable strategy for reconstructing large pediatric defects.
  • This technique addresses tissue deficits in children, promoting normal growth and minimizing donor site complications.
