Kidney Transplant II: Surgical Procedure

827
Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

The Parathyroid Glands00:59

The Parathyroid Glands

6.1K
The two pairs of parathyroid glands embedded within the posterior surface of the thyroid gland are restricted by a dense capsule around them. These glands comprise two distinct cell populations—parathyroid oxyphil and parathyroid principal cells- pivotal in calcium homeostasis.
Oxyphil cells, whose functions remain elusive, emerge during late puberty, adding a layer of complexity to the parathyroid gland's intricacies. In contrast, principal parathyroid cells undertake a vital role by...

Summary

Parathyroid allotransplantation is a safe and effective treatment for hypoparathyroidism, reducing the need for high calcium and vitamin D doses. This procedure improves patients' quality of life with adequate graft survival at 24 months.

Area of Science:

  • Endocrinology
  • Transplant Surgery
  • Neurosurgery

Context:

  • Hypoparathyroidism frequently occurs after neck surgery, necessitating medical treatment with high calcium and vitamin D doses, which carry risks.
  • Current medical management for hypoparathyroidism involves significant complications due to high calcium and vitamin D intake.
  • Parathyroid allotransplantation emerges as a viable alternative management strategy for hypoparathyroidism.

Purpose:

  • To evaluate the safety and efficacy of allogeneic parathyroid transplantation in patients with hypoparathyroidism.
  • To assess graft function and patient outcomes following parathyroid allotransplantation.
  • To determine the long-term viability of parathyroid grafts and their impact on patient quality of life.

Summary:

  • Five patients with iatrogenic hypoparathyroidism underwent parathyroid allotransplantation, with grafts implanted in the forearm under local anesthesia.
  • Four out of five patients showed functional grafts, evidenced by increased parathormone levels and positive scintigraphy results.
  • After a two-year follow-up, functional grafts were observed with reduced oral calcium intake and no immunosuppression side effects.

Impact:

  • Parathyroid allotransplantation offers a safe alternative to medical management for hypoparathyroidism.
  • The procedure successfully decreases reliance on high-dose calcium and vitamin D supplementation.
  • Improved quality of life and adequate graft survival at 24 months were demonstrated, highlighting the procedure's potential.
