Area of Science:

  • Surgical Oncology
  • Minimally Invasive Surgery
  • Endocrinology

Background:

  • Laparoscopic posterior retroperitoneal (PR) adrenalectomy offers an alternative to the transabdominal (TL) approach for adrenal pathologies.
  • Robotic adrenal surgery is noted for improved surgeon ergonomics and enhanced dissection capabilities.
  • PR adrenalectomy may be preferred for patients with bilateral adrenal masses.

Purpose of the Study:

  • To describe the technique of laparoscopic posterior retroperitoneal adrenalectomy.
  • To discuss the limitations associated with the PR approach.
  • To critically review the existing literature on PR adrenalectomy.

Main Methods:

  • Description of the laparoscopic posterior retroperitoneal adrenalectomy technique.
  • Review of current scientific literature on adrenalectomy approaches.
  • Analysis of limitations and benefits of PR adrenalectomy.

Main Results:

  • PR adrenalectomy is a viable alternative to TL adrenalectomy.
  • Robotic assistance may improve outcomes in PR adrenalectomy.
  • The PR approach is particularly suitable for bilateral adrenal masses.

Conclusions:

  • Laparoscopic posterior retroperitoneal adrenalectomy is a valuable surgical option.
  • The technique, especially with robotic assistance, warrants further investigation for adrenal pathologies.
  • Consideration of PR adrenalectomy is recommended for bilateral adrenal disease.
