    Background:

    • Sleep Apnea Syndrome (SAS) affects 5% of the active population globally.
    • Untreated SAS leads to severe comorbidities and increases mortality risk by 39% within eight years.
    • The severity of SAS often escalates without timely diagnosis and intervention.

    Observation:

    • A 49-year-old railroad controller presented with excessive daytime somnolence, sleep-disordered breathing, snoring, headaches, nocturia, and concentration difficulties.
    • Clinical presentation and validated questionnaires (Epworth, Stanford, Berlin) suggested Obstructive Sleep Apnea (OSA).
    • Polygraphy confirmed a severe form of Obstructive Sleep Apnea.

    Findings:

    • The patient was diagnosed with severe Obstructive Sleep Apnea.
    • Immediate management included behavioral therapy (weight loss) and Continuous Positive Airway Pressure (CPAP) therapy.
    • Post-treatment, significant reduction in respiratory symptoms, daytime somnolence, and cognitive difficulties was observed.

    Implications:

    • Early diagnosis and comprehensive management of SAS are crucial for improving patient outcomes.
    • Occupational health settings are vital for identifying and managing sleep-related disorders in workers.
    • CPAP therapy and lifestyle modifications offer effective relief for severe OSA, enhancing quality of life and work performance.
