Background:

Acute appendicitis is a common surgical emergency, with appendectomy being a frequent procedure worldwide.
Anomalies of the appendix, though rare, can complicate diagnosis and surgical management, potentially leading to medico-legal concerns.

Observation:

A 17-year-old female presented with symptoms suggestive of an appendicular lump.
The patient was managed non-operatively with the Ochsner-Sherren regimen, followed by interval appendectomy.
Intraoperatively, a 5 cm appendix with mucosal discontinuity and a connecting fibrous strand was identified.

Findings:

The surgical findings represent a case of appendicular atresia, a rare congenital anomaly.
This is only the fourth reported case of appendicular atresia in the medical literature.
The anomaly involved a complete separation of the appendix's mucosa, with the base attached to the cecum.
