Purpose of the Study:

  • To investigate changes in central arterial hemodynamics throughout gestation.
  • To explore the association between maternal weight and these hemodynamic changes.
  • To assess the impact on cardiac function by term pregnancy.

Main Methods:

  • Longitudinal cohort study of 32 primiparous women.
  • Analyses conducted at 16 and 37 weeks gestation.
  • Utilized 2D speckle tracking echocardiography and tonometric measures for arterial pressures, waveforms, and pulse wave velocity.

Main Results:

  • Cardiac strain decreased from 16 to 37 weeks gestation (P=0.002).
  • This reduction correlated with increased systemic vascular resistance (P=0.008), decreased arterial compliance (P=0.004), and elevated central arterial pressures (P<0.001).
  • Augmentation index also increased significantly (P=0.001).

Conclusions:

  • By 37 weeks gestation, overweight women exhibited higher systemic vascular resistance and central diastolic pressure compared to normal weight women.
  • Overweight women also showed lower ventricular strain at term.
  • Increased systemic vascular resistance and central arterial pressures by term may contribute to cardiac function deterioration, especially in overweight individuals.
