Area of Science:

  • Neurology
  • Sleep Medicine
  • Psychiatry

Background:

  • Investigating the link between untreated obstructive sleep apnea syndrome and neuropsychiatric symptoms.
  • Examining the impact of chronic sleep fragmentation and hypoxia on cognitive and psychiatric function.

Observation:

  • A 30-year-old male presented with intellectual impairment and recurrent psychotic episodes.
  • Polysomnography confirmed severe obstructive sleep apnea syndrome with sleep fragmentation, slow wave sleep deprivation, and arterial oxygen desaturation.

Findings:

  • Tonsillectomy successfully treated obstructive sleep apnea syndrome.
  • Psychotic episodes resolved completely within two years post-treatment.
  • Intellectual impairment persisted despite the resolution of sleep apnea and psychosis.

Implications:

  • Highlights a potential, treatable cause of psychosis in patients with obstructive sleep apnea syndrome.
  • Suggests that while sleep apnea-related psychosis may be reversible, associated intellectual impairment might be persistent.
  • Underscores the importance of screening for sleep disorders in patients with unexplained cognitive and psychiatric symptoms.
