Background:

Endovascular aortic repair (EVAR) is a common treatment for aortic aneurysms.
Open conversion from EVAR can be complex and risky.
The Ovation stent graft's design presents unique considerations for conversion.

Observation:

Two cases of EVAR using the Ovation stent graft requiring open conversion are presented.
One immediate and one late conversion were analyzed.
The specific design features of the Ovation endograft were examined in relation to conversion.

Findings:

The Ovation stent graft's proximal sealing facilitated partial preservation of the endograft.
The preserved endograft components were successfully utilized for anastomosis with a Dacron graft.
This approach proved feasible in both immediate and late conversion scenarios.

Implications:

The Ovation stent graft's design may offer a viable strategy for managing challenging EVAR conversions.
This technique could potentially simplify complex open conversions, improving outcomes.
Further research is warranted to validate this approach in a larger patient cohort.
