Summary

Congestive heart failure can occur in individuals without pre-existing heart conditions due to prolonged atrioventricular tachycardia. Early intervention for supraventricular tachycardia is crucial to prevent myocardial dysfunction and heart failure.

Background:

  • Atrioventricular tachycardia (AVT) can precipitate cardiac events even in individuals without underlying heart disease.
  • Understanding the impact of sustained rapid heart rates on myocardial function is critical.

Observation:

  • A patient without apparent heart disease developed congestive heart failure during a 20-day episode of atrioventricular tachycardia at 180 beats per minute.
  • Symptoms of heart failure rapidly resolved after transesophageal pacing restored normal sinus rhythm.

Findings:

  • Sustained tachycardia at an elevated heart rate may deplete myocardial energy stores, leading to impaired heart muscle function (myocardial dysfunction).
  • This dysfunction manifested as congestive heart failure in the absence of structural heart disease.
