Time for HIV-1/HIV-2 combination tests?

N T Constantine1, J D Callahan, D M Watts

  • 1U.S. Naval Medical Research Unit No. 3 (A World Health Organization Collaborating Center for AIDS), Cairo, Egypt.

Summary

A case of Human Immunodeficiency Virus type 2 (HIV-2) infection was identified in the Middle East, despite negative routine Human Immunodeficiency Virus type 1 (HIV-1) screening. Combined HIV-1/HIV-2 testing is recommended for at-risk individuals.

Area of Science:

  • Infectious Diseases
  • Virology
  • Public Health

Background:

  • Human Immunodeficiency Virus (HIV) infection remains a global health concern.
  • HIV-1 is the predominant subtype, but HIV-2 also circulates, particularly in certain regions.
  • Diagnostic assays are crucial for effective screening and management.

Purpose of the Study:

  • To report a case of HIV-2 infection in the Middle East.
  • To highlight the limitations of HIV-1-specific screening.
  • To advocate for broader HIV screening protocols.

Main Methods:

  • Case report of an individual from the Middle East.
  • Serological testing using routine Enzyme-Linked Immunosorbent Assay (ELISA) for HIV-1.
  • Consideration of HIV-2 specific testing.

Related Experiment Videos

Main Results:

  • An individual tested negative for HIV-1 via routine ELISA.
  • This individual was subsequently identified as infected with HIV-2.
  • The case occurred in the Middle East, a region with varying HIV prevalence patterns.

Conclusions:

  • Routine HIV-1 screening may miss HIV-2 infections.
  • Screening for both HIV-1 and HIV-2 is essential for at-risk populations.
  • Commercially available combination HIV-1/HIV-2 tests offer a solution for comprehensive screening.
