Background:

  • Acquired partial lipodystrophy (APL) is a rare condition with selective fat loss.
  • Metabolic abnormalities were previously considered uncommon in APL compared to other lipodystrophies.

Purpose of the Study:

  • To investigate the prevalence of metabolic abnormalities in Turkish patients with APL.
  • To assess pancreatic beta cell function and hormone levels in APL patients with diabetes.

Main Methods:

  • A nationwide cohort study of 21 Turkish APL patients.
  • Evaluation of metabolic parameters, fat distribution (MRI), hepatic steatosis (ultrasound, MRI, MR spectroscopy).
  • Mixed meal stimulation tests for beta cell function, and measurement of leptin and adiponectin.

Main Results:

  • 71.4% of APL patients exhibited at least one metabolic abnormality.
  • High prevalence of diabetes (28.6%), hypertriglyceridemia (57.1%), low HDL (47.6%), and hepatic steatosis (52.4%).
  • APL patients with diabetes showed insulin resistance, preserved beta cell function, and lower leptin/adiponectin than controls.

Conclusions:

  • APL is associated with a significant risk of developing metabolic abnormalities.
  • Close, long-term monitoring is essential for early detection and management of metabolic complications in APL patients.
