Area of Science:

  • Dermatopathology
  • Vascular Tumors
  • Histology

Background:

  • Hobnail hemangioma is a rare, benign vascular neoplasm.
  • It is characterized by a biphasic histological pattern.
  • This pattern involves dilated superficial vascular spaces and narrow deeper vascular structures.

Observation:

  • A case of hobnail hemangioma presented as a macule with a central papule.
  • Histological examination confirmed the biphasic pattern with hobnail-like endothelial cells.
  • Immunohistochemical analysis showed differential expression of markers in superficial and deeper vessels.

Findings:

  • Superficial vessels expressed CD31, D2-40, and focally Factor VIII.
  • Deeper vessels expressed CD31, CD34, Factor VIII, and α-SMA.

  • The biphasic immunohistochemical pattern suggests a dual origin from lymphatic and blood vessels.
  • Implications:

    • Hobnail hemangioma exhibits a biphasic immunohistochemical profile.
    • This profile supports the hypothesis of a dual origin from both lymphatic and blood vessels.
    • Understanding the immunohistochemical characteristics aids in accurate diagnosis and classification of vascular tumors.
