Summary

Hobnail hemangioma is a rare benign vascular tumor with a distinct biphasic histological pattern. Immunohistochemical analysis reveals a dual origin from both lymphatic and blood vessels.

Background:

Hobnail hemangioma is a rare, benign vascular neoplasm.
It is characterized by a biphasic histological pattern.
This pattern involves dilated superficial vascular spaces and narrow deeper vascular structures.

Observation:

A case of hobnail hemangioma presented as a macule with a central papule.
Histological examination confirmed the biphasic pattern with hobnail-like endothelial cells.
Immunohistochemical analysis showed differential expression of markers in superficial and deeper vessels.

Findings:

Superficial vessels expressed CD31, D2-40, and focally Factor VIII.
Deeper vessels expressed CD31, CD34, Factor VIII, and α-SMA.
