Area of Science:

  • Thoracic Surgery
  • Minimally Invasive Surgery
  • Robotic Surgery

Background:

  • Robot-assisted thoracic surgery (RATS) offers benefits highlighted in North American publications.
  • Establishing a RATS program in a French university setting required a structured approach.
  • This study details the implementation and initial outcomes of RATS in a center with average treatment volume.

Purpose of the Study:

  • To describe the process of establishing a robot-assisted thoracic surgery program.
  • To report the early clinical outcomes of RATS in a French university hospital.
  • To evaluate the safety, efficacy, and learning curve of RATS in a lower-volume center.

Main Methods:

  • Retrospective review of a single institution's database.
  • A 6-month preparation period preceded the program launch.
  • Data collected from January 2012 to January 2013 for 30 patients undergoing RATS.

Main Results:

  • Thirty patients underwent totally endoscopic, full robot-assisted procedures for diverse thoracic pathologies.
  • Median blood loss was 50mL, with a median operating time of 135 minutes.
  • Hospital stay averaged 4 days, with 6 minor complications (20%); all patients were alive at follow-up with good quality of life.

Conclusions:

  • Full robotic thoracic procedures are safe and effective for various pathologies, showing low morbidity.
  • A significant learning curve was not observed, even in a lower-volume center.
  • Robotic technology is crucial for advancing minimally invasive thoracic surgery, emphasizing the need for optimized training and cost-effectiveness.
