A significant aspect of hydroboration–oxidation is the regio- and stereochemical outcome of the reaction.
Hydroboration proceeds in a concerted fashion with the attack of borane on the π bond, giving a cyclic four-centered transition state. The –BH2 group is bonded to the less substituted carbon and –H to the more substituted carbon. The concerted nature requires the simultaneous addition of –H and –BH2 across the same face of the alkene giving syn stereochemistry.

Background:

  • Spontaneous esophageal rupture is a rare but life-threatening condition.

Purpose of the Study:

  • To report a rare case of recurrent spontaneous esophageal perforation.
  • To contribute to the literature on managing and understanding this infrequent event.

Main Methods:

  • A 43-year-old male patient presented with symptoms suggestive of esophageal rupture.
  • Diagnostic workup included esophagoscopy, esophagogram, and computed tomography.
  • Surgical intervention involved left thoracolaparotomy for repair and drainage.

Main Results:

  • A 20-mm perforation in the left lower esophagus, previously repaired, was identified and surgically closed.
  • The patient recovered well following the repair and drainage procedures.
  • This represents one of the few reported cases of recurrent spontaneous esophageal perforation.
