    Summary

    Transcatheter aortic valve replacement (TAVR) via transapical approach is effective for severe aortic stenosis in high-risk patients. This study reports initial clinical outcomes, showing a 90.7% device success rate.

    Background:

    • Severe aortic stenosis poses a significant risk for conventional surgical therapy.
    • Transcatheter aortic valve replacement (TAVR) offers an alternative for high-risk patients.
    • The transapical approach is an accepted TAVR method.

    Purpose of the Study:

    • To report the initial clinical results of transapical TAVR in a reference center.
    • To evaluate the safety and efficacy of this TAVR approach in high-risk patients.

    Main Methods:

    • A prospective, single-center study of 54 consecutive patients undergoing transapical TAVR between November 2008 and October 2013.
    • Patients had severe aortic stenosis, with high logistic EuroSCORE (19.8 ± 11.2) and EuroSCORE II (5.5 ± 3.5%).
    • Outcomes were assessed using Valve Academic Research Consortium-2 (VARC-2) criteria.

    Main Results:

    • 30-day mortality was 5.6%.
    • Key VARC-2 endpoints included device success (90.7%), early safety (75.9%), and 30-day clinical efficacy (83.7%).
    • Complications included peri-procedural myocardial infarction (7.4%) and major vascular complications (5.6%).

    Conclusions:

    • The transapical approach is an effective therapy for patients with severe aortic stenosis who are at high risk for conventional surgery.
    • Initial results demonstrate acceptable safety and efficacy profiles for this TAVR technique.
