• This study reviewed 22 pediatric patients with spleen injuries from 1970-1988.
• All patients underwent splenectomy, with 3 cases of autotransplantation and 1 of surgical conservation.
• One death occurred due to polytrauma; infection rates were low.

Impact:

• Splenectomy is a viable, life-saving option for pediatric spleen injuries when preservation is not feasible.
• Surgical spleen conservation should be attempted only when conditions permit.
• Children can live healthy lives without a spleen.
