Anatomy of the Adrenal Glands01:17

Anatomy of the Adrenal Glands

6.1K
The adrenal or supra-renal glands, situated above the kidneys and aligned with the twelfth rib, are paired pyramid-shaped structures crucial for the body's stress response. During stress, these glands secrete hormones vital for adaptive physiological reactions.
These glands possess a distinctive yellow tinge due to the stored cholesterol and fatty acids required for hormone synthesis. They are encased in a fibrous capsule and cushioned by fat.
The adrenal gland comprises two distinct...

Endoscopic Studies I: Bronchoscopy and Thoracoscopy

931
Endoscopy is a non-surgical medical technique used to examine a person's internal organs and vessels. This lesson will focus on two types of endoscopic studies: bronchoscopy and thoracoscopy.
Bronchoscopy
Description
Bronchoscopy is a procedure that involves direct visualization of the larynx, trachea, and bronchi for diagnostic and therapeutic purposes. A flexible fiber optic or rigid bronchoscope is used to carry out the procedure. The fiber-optic bronchoscope is more frequently used due...

Endoscopic Studies II: Thoracocentesis

2.1K
Thoracentesis(Thoracocentesis), commonly known as pleural tap, is a medical procedure where a 22 gauge needle is inserted into the pleural space, the area between the lung and chest wall. This procedure is commonly performed to diagnose or treat various respiratory disorders.
Description
Excess pleural fluid or air may accumulate in some respiratory disorders in the thoracic cavity. To treat pleural effusion, a physician conducts thoracentesis by carefully piercing the chest wall and entering...

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

Endoscopic Procedures V: ERCP

6.0K
Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Summary

Chimney endovascular sealing (chEVAS) offers a solution for complex abdominal aortic aneurysms when other methods fail. This technique successfully excluded aneurysms in two challenging cases, but long-term durability remains unknown.

Area of Science:

  • Vascular Surgery
  • Endovascular Repair
  • Aortic Aneurysm Treatment

Background:

  • Endovascular sealing with Nellix (EVAS) is an emerging technique for infrarenal abdominal aortic aneurysms.
  • Conventional endovascular repair (EVAR) has limitations in certain complex aortic anatomies.

Observation:

  • This report details the use of endovascular sealing combined with chimney stents for renal arteries (chEVAS) in two patients.
  • Case 1: Refractory type Ia endoleak and expanding aneurysm.
  • Case 2: Large juxtarenal aneurysm unsuitable for standard EVAR.

Findings:

  • Both patients treated with chEVAS achieved successful aneurysm exclusion.
  • The technique proved effective in managing complex cases requiring suprarenal seal.

Implications:

  • Chimney EVAS demonstrates utility in challenging abdominal aortic aneurysm cases where standard EVAR or open repair is difficult.
  • Further research is needed to establish the long-term durability of chEVAS before widespread adoption.
