Pulmonary hypertension (PH) affects 18% of COPD patients, particularly those with severe airflow limitation and hypoxemia. Exercise-induced PH is common, even in moderate COPD, potentially limiting exercise tolerance.

- PH prevalence is similar in severe and very-severe COPD and linked to arterial hypoxemia.
- Exercise-induced PH is highly prevalent, even in moderate COPD, and may impair exercise capacity.
- These findings highlight the importance of screening for PH in COPD patients, especially those with hypoxemia.
