    Background:

    • Morbid obesity is a growing public health concern.
    • Obesity is increasingly associated with metabolic syndrome and nonalcoholic fatty liver disease (NAFLD).
    • NAFLD encompasses a spectrum of liver conditions, from simple steatosis to steatohepatitis and cirrhosis.

    Purpose of the Study:

    • To investigate correlations between serum biochemical markers and liver histology in morbidly obese patients.
    • To highlight the association between obesity, metabolic syndrome, NAFLD, and hepatocellular carcinoma (HCC).
    • To emphasize the need for identifying at-risk populations for NAFLD progression and HCC development.

    Main Methods:

    • A retrospective clinical study involving 13 morbidly obese patients undergoing bariatric surgery.
    • Liver biopsy was performed during surgery to assess liver histology.
    • Patients without other risk factors for liver disease were included.

    Main Results:

    • All 13 patients exhibited varying stages of NAFLD, including steatohepatitis (7), steatosis (5), and evolving cirrhosis (1).
    • Classical liver function tests were normal in all patients except the one with evolving cirrhosis.
    • No significant correlation was found between biochemical markers and liver pathology, except in advanced cirrhosis.

    Conclusions:

    • Hepatic alterations in obese patients may not be reflected in standard liver function tests.
    • Adipokines secreted by adipose tissue are implicated in NAFLD development and progression.
    • Further research is needed to elucidate the pathway from fatty liver to HCC and identify at-risk individuals.
