Background:

  • Dynamic contrast-enhanced MRI (DCE-MRI) evaluates tumor microvascular perfusion and permeability.
  • Comparability of DCE-MRI measurements across centers is uncertain due to methodological variations.

Purpose of the Study:

  • To assess the impact of common analytical methods on DCE-MRI biomarker values.
  • To evaluate how these methods affect the repeatability of DCE-MRI measurements.

Main Methods:

  • DCE-MRI was performed on rats with C6 xenografts over three days.
  • Analyzed biomarkers included Ktrans, ve, and kep using 2- vs. 3-parameter models, individual vs. population VIFs, and pixel-by-pixel vs. whole-tumor ROI.
  • Variability was quantified using within-subject coefficient of variation (wCV).

Main Results:

  • Absolute DCE-MRI parameter values and wCVs varied significantly based on analytical methods.
  • Ktrans and kep were lower with 3-parameter modeling; kep and vp were lower with pixel-wise analysis.
  • Individual VIFs showed smaller wCVs for ve but larger wCVs for kep compared to population VIFs.

Conclusions:

  • DCE-MRI biomarker values and repeatability are highly dependent on analytical methodology.
  • Lack of standardization in DCE-MRI analysis hinders comparability of results between studies.
