Mortality and flexible ureteroscopy: analysis of six cases

Retrograde intrarenal surgery (RIRS) is a common kidney stone treatment with low complication rates. However, rare fatal outcomes, particularly in patients with prior infections, can still occur despite adherence to care standards.

Background:
- Retrograde intrarenal surgery (RIRS) is a widely adopted technique for kidney stone treatment, comparable to traditional methods.
- Despite its generally low complication rate, fatal outcomes following RIRS are not well-documented.
Observation:
- A survey of 11 experienced endourologists identified six reported cases of mortality associated with RIRS.
- Three of these fatal cases involved patients with a history of urinary tract infections.
Findings:
- The primary causes of death were urosepsis (four cases), anesthetic complications (one case), and hemorrhagic complications (one case).
- The use of ureteral access sheaths was infrequent in the reported fatal cases.
Implications:
- While RIRS is a safe and effective option for most kidney stones, rare fatal complications can occur, especially in complex cases or those with pre-existing conditions like urinary tract infections or advanced neurological diseases.
- Adherence to safety protocols is crucial, even when performing established procedures like RIRS.
