    Area of Science:

    • Neurology
    • Movement Disorders

    Background:

    • Orthostatic tremor is a rare condition characterized by rhythmic muscle activity that occurs when standing.
    • Essential tremor is a common neurological disorder that causes involuntary shaking.

    Observation:

    • A 62-year-old male presented with tremors of the trunk and lower limbs exclusively upon standing.
    • The tremor exhibited a frequency of 14-16 Hz with co-contraction of antagonistic muscles.

    Findings:

    • Standard medications including propranolol, primidone, and diazepam provided no therapeutic benefit.
    • The case highlights a rare presentation of orthostatic tremor.

    Implications:

    • Further research into the pathogenesis of this rare orthostatic trunkal tremor is warranted.
    • Understanding its relationship with essential tremor may offer new diagnostic or therapeutic insights.
