Background:

  • Osteochondroma is a common benign bone tumor, but extraskeletal variants are rare.
  • Extraskeletal osteochondroma has three variants: synovial chondromatosis, para-articular chondroma, and soft-tissue chondroma.

Observation:

  • A 78-year-old female presented with progressive knee pain and severe arthrosis.
  • An intra-articular lesion in Hoffa's fat pad was identified.
  • Clinical and radiological findings suggested para-articular osteochondroma.

Findings:

  • Histopathological examination after lesion excision revealed synovial chondromatosis.
  • The synovial chondromatosis was secondary to underlying osteoarthrosis.
  • This contrasts with the initial clinical and radiological diagnosis.

Implications:

  • This case underscores the importance of histopathological confirmation for diagnosing infrapatellar fat pad lesions.
  • It highlights synovial chondromatosis as a potential diagnosis in cases of knee pain associated with arthrosis.
  • Accurate diagnosis is crucial for appropriate management of knee joint pathologies.
