Summary

Chronic renal failure patients with severe secondary hyperparathyroidism developed pseudoclubbing. While treatments improved radiological signs, finger deformities persisted, indicating parathormone autonomy is not always present.

Background:

  • Chronic renal failure (CRF) is associated with complex metabolic disturbances.
  • Secondary hyperparathyroidism is a common complication of CRF, involving elevated parathormone (PTH) levels.

Observation:

  • Seven patients with CRF presented with a unique digital abnormality termed pseudoclubbing.
  • Radiological assessments revealed severe secondary hyperparathyroidism in all patients.
  • Elevated C-terminal parathormone levels were consistently measured.

Findings:

  • Despite successful treatment interventions including vitamin D, calcium, parathyroidectomy, and renal transplantation, radiological evidence of hyperparathyroidism healed.
  • However, the fingertip deformities (pseudoclubbing) did not show improvement post-treatment.
  • This suggests that autonomous parathormone secretion is not an obligatory feature of severe secondary hyperparathyroidism in CRF.

Implications:

  • Pseudoclubbing may be a distinct clinical manifestation of severe secondary hyperparathyroidism in CRF patients.
  • The persistence of pseudoclubbing despite treatment highlights potential limitations in reversing all manifestations of severe hyperparathyroidism.
  • Further research is needed to elucidate the specific mechanisms underlying pseudoclubbing and its relationship with PTH levels and renal disease.
