Summary
Patients with retroperitoneal teratoma treated with primary retroperitoneal lymph node dissection (RPLND) alone have a low relapse rate. Adjuvant chemotherapy is not recommended for these patients.
Area of Science:
- Oncology
- Urology
- Surgical Pathology
Background:
- Retroperitoneal teratomas are rare germ cell tumors.
- Primary retroperitoneal lymph node dissection (RPLND) is a key treatment modality.
- The role of adjuvant chemotherapy in teratoma-only RPLND remains debated.
Purpose of the Study:
- To assess oncologic outcomes in patients with teratoma-only retroperitoneal tumors.
- To evaluate the necessity of adjuvant chemotherapy after primary RPLND.
- To determine relapse rates based on pathologic stage.
Main Methods:
- Retrospective review of 23 patients with teratoma-only retroperitoneal tumors.
- All patients underwent primary RPLND between 1979 and 2010.
- No patients received adjuvant chemotherapy; minimum follow-up was 2 years.
Main Results:
- 100% 5-year disease-free survival (DFS) with a median follow-up of 5.8 years.
- No significant DFS difference between pathologic stages IIA and IIB/IIC (P = .73).
- Two late relapses (14%) occurred 11 years post-RPLND, treated successfully.
Conclusions:
- The relapse rate for teratoma-only RPLND is low, regardless of pathologic stage.
- Adjuvant chemotherapy is not recommended in the management of these patients.
- Careful surveillance is crucial for early detection of late recurrences.
