Summary

A modified salt solution (MSS) effectively prevented bacterial regrowth in root canals, outperforming calcium hydroxide (Ca(OH)2) in preventing persistent Enterococcus faecalis infections after disinfection.

Background:

  • Persistent Enterococcus faecalis infections pose a challenge in endodontic treatment.
  • Sodium hypochlorite (NaOCl) is a standard irrigant, but additional disinfection methods are explored.
  • Calcium hydroxide (Ca(OH)2) is commonly used as an intracanal medicament.

Purpose of the Study:

  • To evaluate the disinfecting properties of a modified salt solution (MSS) and Ca(OH)2.
  • To compare their efficacy against Enterococcus faecalis in a non-direct-contact ex-vivo model.
  • To assess their ability to prevent bacterial regrowth after initial disinfection.

Main Methods:

  • Seventy-four infected root canals were treated with NaOCl irrigation, with or without subsequent MSS or Ca(OH)2 dressing.
  • Bacterial regrowth was assessed after incubation by sampling root canals.
  • Colony-forming units were counted, and regrowth was determined (no/yes).

Main Results:

  • MSS dressing significantly reduced bacterial regrowth compared to Ca(OH)2 after NaOCl irrigation (P=0.009).
  • After NaOCl irrigation alone, 14/19 roots showed regrowth.
  • MSS dressing resulted in regrowth in 6/20 roots, while Ca(OH)2 showed regrowth in 14/20 roots.

Conclusions:

  • The modified salt solution demonstrated effectiveness in preventing bacterial regrowth, indicating potential for eliminating persistent root canal bacteria.
  • Intracanal dressing with Ca(OH)2 did not offer additional disinfection benefits beyond NaOCl irrigation.
  • MSS shows promise as an adjunctive disinfection agent in endodontics.
