    Summary

    Background:

    • Lithium is a widely prescribed mood stabilizer for bipolar disorder.
    • Long-term lithium use is associated with potential adverse effects, particularly renal and endocrine complications.

    Purpose of the Study:

    • To evaluate the renal, thyroid, haematological, cardiac, and endocrine effects of long-term lithium therapy.
    • To identify specific functional deficits in patients undergoing chronic lithium treatment.

    Main Methods:

    • Ten patients on long-term lithium therapy underwent comprehensive assessments.
    • Tests included renal function, urinary concentrating ability (with vasopressin stimulation), metabolic acidosis evaluation, thyroid function tests, electrocardiograms (ECGs), and haematological profiling.

    Main Results:

    • Seven out of ten patients exhibited impaired urinary concentrating ability, indicative of partial nephrogenic diabetes insipidus.
    • Nine patients presented with metabolic acidosis and elevated urinary pH, suggesting renal acidification defects.
    • No significant alterations were observed in renal function, thyroid function, ECG parameters, or haematological indices.

    Conclusions:

    • Long-term lithium therapy is associated with functional renal abnormalities, including impaired concentrating ability and acidification defects.
    • These findings suggest a potential for subclinical nephropathy in patients on chronic lithium treatment.
    • Despite these functional changes, overall renal, thyroid, cardiac, and haematological safety profiles appear favorable, consistent with existing literature.
