Summary

Major Depressive Disorder (MDD) is linked to hypothalamic-pituitary-adrenal (HPA) axis dysregulation. Poor cortisol suppression after dexamethasone correlates with impaired verbal memory in MDD patients.

  • The hypothalamic-pituitary-adrenal (HPA) axis plays a crucial role in the stress response and is frequently implicated in MDD.

Purpose of the Study:

  • To investigate the relationship between cortisol levels, measured via the dexamethasone suppression test, and cognitive function in patients with recurrent unipolar Major Depressive Disorder.
  • To assess HPA axis regulation and its impact on cognitive domains in MDD.

Main Methods:

  • Twenty-four patients with recurrent MDD and a matched control group participated.
  • Salivary cortisol was measured four times daily for two days, with dexamethasone administered on Day 1.
  • Cognitive function was assessed using a battery of neuropsychological tests measuring verbal memory, visual memory, and executive functions.

Main Results:

  • Cortisol levels did not differ between groups on Day 1 but were higher in MDD patients at awakening on Day 2 after dexamethasone.
  • Negative correlations were observed between cortisol levels and cognitive measures, indicating poor suppression is linked to impaired cognition.
  • Significant associations were found between cortisol levels and verbal memory, semantic fluency, and inhibition.

Conclusions:

  • HPA axis dysregulation, evidenced by poor cortisol suppression, is associated with impaired verbal memory functioning in Major Depressive Disorder.
  • While associations were found with verbal memory, firm evidence linking abnormal cortisol levels to inhibition difficulties was not established.
